To schedule your Class D Knowledge Exam (Permit Test) please visit:

Age: Must be 15 years of age
Documentation Needed: 

• Blue Card

• Parent/Guardian 

• Know your social security number

• Permit fee of $15.50 cash/check only
  
  

To schedule your Class D Skills Exam - First Time (Road Test) please visit:
Road Test

Age: Must be 16 years of age
Documentation Needed:

• White Card

• Parent/Guardian

• Permit (must have held permit for at least 6 months) 

• Know your social security number

• Proof of Insurance

• Completed Driving Log (50 hours / 15 at night - 40 hours / 1

• Certificate from Point of Impact Parent Meeting (if attended) 

• Class D license fee - bring $32 cash/check

Retesting Fees:

• Third and subsequent knowledge test (after failing first two) - $10.00

• Third and subsequent road/skills test (after failing first two) - $20.00
